ALL FIELD TRIPS

This form should be filled out completely and accurately. Failure to provide all required information may result in a denial of your request.

Must submit their requests at least two weeks prior to the trip. Failure to adhere to these timelines will result in a denied request.

You must include a tentative roster of students. It is understood that the roster may change.

You must include a detailed agenda of activities and their locations.

If a field trip occurs during school hours and results in students being off campus during lunch, the school/teacher is required to coordinate meals through the cafeteria. Arrangements must be made with the cafeteria manager at least two weeks prior to the field trip date. This information must be documented in the “Provisions for Meals” section of the field trip request.
TRIPS MORE THAN 50 MILES AWAY (one way)

Required submission of turn-by-turn directions. Failure to provide these directions may result in a denial of your request.

Must submit requests at least four weeks prior to the trip date. Failure to adhere to these timelines will result in a denied request.
TRIPS WITH OVERNIGHT STAYS

Must submit their requests at least four weeks prior to the trip date. Failure to adhere to these timelines will result in a denied request.
