Annual Evaluation of Title I Parent and Family Engagement Plan

Dear Parents/Guardians,

We at Hawkeye Elementary School need your help planning our school policies.  Please take a few minutes to review the attached Parent and Family Engagement Plan that outlines how parents and family members may be involved and the Compact that lists the responsibilities of the school, parents, and students.  Then, fill out the bottom portion of this form and return it to your child’s teacher.  Thank you for your help and cooperation.

Nosotros en Hawkeye Elementary School necesitamos su ayuda para planificar nuestras políticas escolares.  Por favor tómese unos minutos para revisar el Plan de Participación de Padres y Familias adjunto que describe cómo los padres y los miembros de la familia pueden estar involucrados, y el Pacto que enumera las responsabilidades de la escuela, los padres y los estudiantes.  Luego complete la parte inferior de este formulario y devuélvalo al maestro de su hijo/a.  Gracias por su ayuda y cooperación.
