    Parent/Guardians:

     

    You are receiving this letter because when your child(ren) registered with Hoke County Schools, you indicated that they are American Indian/Alaska Native or Multi-ethnic with one parent being  American Indian. Hoke County Schools system applies for and receives funds from the United States Office of Indian Education each year. These funds are used for direct educational services for Native American students enrolled in our school system. 

     

    The ED 506 form or the Title VI Indian Student Eligibility Certificate form is

    the required form that needs to be filled out. The United States Office of Indian Education requires that school districts have a completed form on file for each student enrolled who identifies as being American Indian/Alaska Native or Multi-ethnic with one parent being  American Indian. Completion of this form ensures that funding is granted to the school system and enrolls your child(ren) in the Indian Education programs available throughout Hoke County Schools.  It will also fulfill the requirements for your child(ren) to be part of the Project Indigechoice through the Lumbee tribe (https://indigechoice.lumbeetribe.com). 

     

    Below is the information that must be completed on the ED 506 form or the Title VI Indian Student Eligibility Certificate form by the parent or legal guardian of the student. One form per child must be completed and submitted back to the school.

    The 506 form MUST be completed entirely for your child to be enrolled.

      1. Student Information- List your child’s name, date of birth, grade level, school name and district (Hoke County Schools). This information is required. 
      2. Tribal membership- Please select who has a tribal membership (child, parent or grandparent) and include the members name. If no tribal membership, leave blank.
      3. List the name of the tribe or band that your student identifies as. (Example: Lumbee- state recognized tribe, Cherokee, Waccamaw- Siouan, etc.) This information is required. 
      4. Include tribal membership number or birth certificate copy- Write the tribal membership number on this form. If you do not have a tribal enrollment number, you can attach a copy of the child’s birth certificate, listing at least one parent with Native American raceDocumentation is required.
      5. Attestation Statement- Make sure you sign and date the form at the bottom and give us a current cell number and email address. Signature & date are required.
